Cette observation n’est pas une histoire de chasse !
Patient de 80 ans, violente douleur abdominale , puis accalmie relative.
Ventre sensible mais qui ne se défend pas , une échographie est réalisée au lit du malade : sonde positionnée en zone épigastrique , juste tenue par le fil , sans comprimer la peau ++++
On retrouve une image typique de pneumopéritoine , avec son air extra luminal représenté par cette zone linéaire hyperéchogène et son magnifique artefact de répétition et signe du code barre au TM (rappelant le pneumothorax ! ) .
Diagnostic : perforation d’organe creux (le Tdm puis la laparotomie exploratrice confirmeront la perforation gastrique )
Le secret ?
Utilisez une sonde Haute Fréquence
Posez simplement la sonde SANS appuyer (un peu de douceur ! )
Mettez une bonne dose de gel
Tenez la sonde par son câble en effleurant la peau
Ne pas comprimer les tissus , ce qui chasserait du coup la bulle d’air
Explorer l’épigastre +++ (l’air monte ++++ )
Bien faire la différence entre l’air libre péritonéal de l’air endoluminal !
